Our consultation services are designed to provide one-on-one support in person or virtually for a variety of health-related topics, giving you time to discuss your questions, concerns, and goals with our pharmacist.

To request a consultation, please complete the form below. Before your appointment, you will be asked to complete and submit the appropriate intake form so our team can review your information and prepare for your consultation. A consultation fee applies.

Once your request is received, a member of our team will follow up with the next steps, scheduling information, fee, and any forms needed before your appointment.
